Healthcare management case scenario-analysis

The project related to the composition of the board, including an evaluation of its governance programs and processes. A board member confided a potential problem. Apparently, a cardiology group owned a cardiac catheter laboratory housed in the hospital.
This arrangement was not included in any governance documents that I had reviewed.

I looked at some utilization data. Admittedly, the data were not comprehensive, and I do not possess the expertise to interpret them properly. Nevertheless, it seemed evident that not all patients met the criteria for some of the cardiac tests and procedures.

I raised the issue with a long-term member of board that since the lab was owned by the physicians who ordered the tests, this could surely constitute a conflict of interest; moreover, the hospital’s — and most particularly, the board’s — culpability could not be ruled out given the lab’s presence on its premises and that the technicians were employees of the hospital.

The board member indicated that this had been a considerable dilemma for some time. The cardiac group provided substantial business to the hospital, and this lab was viewed as a sort of “perk” for the group. The lab reimbursed the hospital for the employees’ compensation and benefits; however, no fees were paid for rent or for any other resources used in the lab, for example, for equipment.

The board member indicated the arrangement was good for all, and the cost to the hospital was more than made up by the patients that were brought to the hospital by the cardiac group. Finally, the board member indicated that if it challenged or sought to undo the arrangement, the cardiac group, which was highly sought by other hospitals, would probably leave.

Based on the reading and your understanding of the conflict between ethical, legal, political, and financial interests, what are the key factors influencing this situation? What are the risks of the hospital taking no action? What recommendations would you make that take all the key factors into consideration?
