Conclusions and Recommendations –
This is a summary of the key points that you have raised within the essay. Demonstrate that you have explored the patient journey as set out in the introduction. Identify how the new learning gained will inform your future practice in relation to providing a holistic approach to long term conditions. (approx. 350 words)
