  • 55, item (b)(3)(ii) states that a facility’s security program must “provide defense in depth.” With regard to facility security, what components would be included in “defense in depth”?
  • 55, item (b)(9)(i) describes an “insider mitigation” program that, “must monitor the initial and continued trustworthiness and reliability of individuals granted unescorted access to a protected or vital area” (USNRC, 2012, para. 18). What efforts do you think are required to ensure trustworthiness and reliability of individuals?

What is the purpose of this statement?
  • Appendix B, Section XVI, Corrective Action, states that: “Measures shall be established to ensure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, etc. , are promptly identified and corrected” (USNRC, 2012, para. 20). How do you think human performance (that is, performance of reactor operators, technicians, etc. ) would be addressed by the corrective action process?
